Support Coordination at Support With Tender Care
We currently offer the following levels of Support Coordination:
Level 1 — Support Connection
Support Connection is a short-term and lower-intensity support designed to help participants:
- Understand their NDIS plan
- Connect with service providers and community supports
- Access mainstream services such as health, education, and community programs
- Develop confidence in using their NDIS funding
The focus is on helping participants get started with their supports and build their capacity to manage services independently.
Level 2 — Coordination of Supports
Support Coordination is a higher level of assistance for participants with more complex support needs. A Support Coordinator works closely with the participant to:
- Implement and monitor their NDIS plan
- Coordinate multiple service providers
- Resolve service delivery issues
- Build capacity and independence
- Develop informal, community, and mainstream support networks
- Prepare for plan reviews and support changes in circumstances
- Ensure services are working together to achieve the participant’s goals
Support Coordinators help participants make informed decisions, maximize their NDIS funding, and achieve greater independence and community participation.
